For Patients - About Your Visit

On your first visit please arrive 15 minutes prior to your appointment time and have the provided you with forms that need to be completed. If your insurance requires a referral please bring it with you to your appointment.

Also have the following items with you:

  • Valid Drivers License or State ID
  • Insurance card(s)
  • List of medications you take on a regular basis, including over-the-counter
  • medications and vitamins.
  • List of surgeries or medical procedures you have had
  • Payment for your co-pay or any non-covered services. For your convenience,
    we take cash, check, VISA, MasterCard, and Discover.

Financial Policy:

  • All copays, deductibles, and non-covered service charges are due at the time of service.
  • There is a $10 per month statement fee for all balances over 30 days.
  • There is a $5 fee to complete forms for disability, insurance, FMLA, etc., which is required before the forms can be completed.
  • NSF checks are charged $35.

The greatest compliment our practice can receive is the referral of friends and family. If you were referred by someone, please let us know who so that we may thank them. If you refer someone, as an expression of our appreciation we will send you a gift card after their first appointment.

Please contact us with at least 24 hours notice if you need to cancel or reschedule your appointment. There may be a $25 charge for missed appointments without 24 hour notice of cancellation. If you arrive more than 15 minutes late we may need to reschedule you as to not delay following appointments.
